Jeffrey A. Alper, M.D. CPI is a Board-Certified Rheumatologist and Internal Medicine with 35 years of clinical experience.  Dr. Alper became interested in clinical research in the late 1990’s and has served as the Principal Investigator for more than 50 studies including novel new therapies for autoimmune disease.  Dr. Alper obtained the Certified Principal Investigator designator through ACRP in 2016.  He is a native of Miami, where he obtained his medical  degree at the prestigious University of Miami. For his residency, Dr. Alper completed his medical       internship at Jackson Memorial Hospital in Miami and his Internal Medicine and Rheumatology Fellowship at Medical University of South Carolina. Dr. Alper has been married for 42 years, has 2 grown children and 1 grandchild.

Alain Alvarez, M.D. 


Bilingual in English and Spanish, Dr. Alvarez was born in Havana, Cuba and raised in Miami, Florida. Dr. Alvarez graduated Phi Kappa Phi with both his Bachelor of Science degree in microbiology and immunology and his medical degree from the University of Miami. He completed his residency in internal medicine at The Mayo Clinic in Jacksonville, Florida and his Rheumatology fellowship training at the University of Minnesota.  Dr. Alvarez is board certified in Rheumatology and Internal Medicine.

Dr. Alvarez has a keen interest in providing non-surgical orthopedic alternatives to his patients. He also holds a special interest in musculoskeletal ultrasound for which he has undergone additional training to be certified both nationally and internationally in the application of this technology for the treatment of rheumatic diseases. 

Terry Smith, ARNP


Terry Smith is an Advanced Registered Nurse Practitioner who moved to this area from New York State. She graduated as a Certified Family Nurse Practitioner in 1997 from Community General Hospital in Syracuse, NY.  Terry has participated in over a dozen studies at MCRI including novel new therapies for autoimmune disease with Dr. Alper.

Prior to Terry moving to Naples, FL and joining the MCRI team, she owned and operated a successful internal medicine practice.   She has more than 15 years of experience in diverse clinical settings.  Terry is known for her comprehensive assessments and diagnostic services.
